Hoarding Case Manager

Overview

Catholic Charities offers a Hoarding Intervention and Treatment Program, the only program of its kind in the nation, aimed at helping individuals with hoarding disorder to improve their living spaces and overall well‑being.

• Conduct client inquiries and in‑home assessments to understand clients’ physical, social, and financial needs.
• Collaborate with Home Coaches and Therapists to develop individualized care plans.
• Assist clients one on one in their home with environmental de‑cluttering, organization, and other support as outlined in the care plan.
• Help clients access essential services (e.g., Medicare, food assistance, family care, disability programs).
• Monitor client progress, evaluate treatment outcomes, advocate for additional resources, and provide personal support during crises.

• Bachelor’s degree in Social Work, Psychology, Sociology, or a related field.
• At least six months of relevant experience.
