Case Worker/Social Worker – Children’s Long-Term Support
The position plays a vital role in helping children and families access services, supports, and resources that promote safety, stability, and well‑being. The role includes providing compassionate guidance in person and by phone, coordinating referrals, collaborating with community partners, and delivering CCS and/or CLTS services in alignment with Wisconsin program requirements. Documentation, functional screenings, and support of families through a range of needs are also part of the responsibilities.
Ideal candidates are organized, empathetic, and energized by helping children and families thrive.
- Provide assistance over the phone or in person to the public during all business hours.
- Receive phone, in‑person or electronic reports, referrals and requests for services and resources.
- Make consumer referrals; maintain contact with other agencies/organizations; coordinate services with a variety of providers and family members.
- Complete accurate and timely record‑keeping including case notes, assessments, treatment plans, crisis plans, court recommendations and billing activities.
- Attend staff training and other meetings as required.
- Complete responsibilities within statutory or administrative rule timelines and meet monthly productivity documentation or billing goals.
- Other duties as assigned.
- Mandated by Wisconsin Administrative Code DHS 36 (CCS) or the Wisconsin Medicaid Home and Community‑Based Waiver Manual (CLTS).
- Provide CCS and/or CLTS community services per Wisconsin statutes, court policies, and/or program, state or federal guidelines.
- Provide services in support of recovery concepts and practices.
- Explain the scope of services and discuss consumer’s rights and responsibilities.
- Perform functional screening for publicly funded services.
Education and Experience
- Bachelor’s degree in Social Work, Human Development and Family Studies, Psychology, Sociology, or related field.
- Minimum of one (1) year previous experience in Social Work or Human Services.
- Valid driver’s license or ability to travel between work locations.
- Background check prior to hire.
- Obtain and maintain Functional Screener Certification upon hire.
- Pass the Continued Skills Test administered bi‑annually.
- Complete NASW Code of Ethics training and read MPSW Code of Conduct within six months of hire.
- Certified Social Worker (CSW) or possession of a social worker training certificate.
- Certified Advanced Practice Social Worker (CAPSW).
- Certified Independent Social Worker (CISW).
- Licensed Clinical Social Worker (LCSW).
- Master’s degree in social work, human services, education, counseling or psychology.
- Minimum of two (2) years previous experience in Human Services or Social Work.
- Licensed Professional Counselor (LPC).
- Marriage & Family Therapist (MFT).
- Knowledge of local and state regulations, rules, policies pertaining to specific program assignments.
- Knowledge of fundamentals of social services, family systems, child development, therapy, intervention, chemical dependency, abuse, neglect, family violence, aging, and disability benefits.
- Knowledge of interviewing, assessing, counseling and evaluating.
- Knowledge of community resources and needs.
- Knowledge of case management and record‑keeping requirements.
- Skill in assessing client issues and making appropriate referrals.
- Skill in applying professional judgment, discretion, and decision making to develop strategies and plans.
- Skill in investigating, interviewing & gathering information for assessments and evaluations.
- Skill in developing individual service plans and coordinating consumer services.
- Skill in preparing court studies, documents and testifying in court.
- Skill in conducting child, elderly or vulnerable adult abuse/neglect investigations.
