* Job Summary:

** As an integrated member of the healthcare team, the goal is to improve health outcomes by serving as the liaison between the clinical setting and the community. This role empowers underserved patients and families by identifying and utilizing resources to support achievement of whole child and family health goals, working towards long term sustainability and independence. Serves as a facilitator among patient families and internal & external resources and demonstrates strong relationship building and problem-solving skills.

* Essential Functions:

*** Engages families in motivational dialogue to determine best resource options and creates an action plan to obtain these resources to support families.
* Supports families in identifying and clarifying basic resource needs (food, housing, childcare, etc.) in the outpatient clinic setting.
* Conducts ongoing telephonic and clinic-based visits to address the healthcare and social drivers of health (SDOH) of children and caregivers, while following up with families to assess the effectiveness of referrals and provide additional support as needed.
* Operates with continual and intentional integration with expanded care team and system partners to address SDOH and other programming needs to optimize care coordination and resource provision.
* Collects and documents information pertaining to identified needs of patients and/or families and of the services provided in a systematic, continuous, and confidential manner in the EHR.
* Navigates the needs of patients, caregivers, and staff by prioritizing interventions across multiple sectors.
* Supports and contributes to the development and evaluation of Health Navigation program goals.
* Evaluates and advises Health Navigation program on the quality and effectiveness of identified resources, while monitoring and alerting the team to emerging trends, seasonal disruptions, or community barriers. In such cases, collaborates to ensure that additional support is provided to wrap around the patient and family to address their needs effectively.

* Education:

*** High School graduate or Certificate of General Educational Development (GED) or High School Equivalency Diploma (HSED) required
* Associate's Degree in human services, public health, or related field preferred
*
* Experience:

*** 2+ years working with underserved/vulnerable population required
* Experience working with families of diverse ethnic, cultural and socioeconomic backgrounds required
* Experience navigating EHR/Epic preferred
*
* Knowledge, Skills and Abilities:

*** Community Health Worker certification required within 12 months of employment
* Ability to work independently
* Working knowledge of Windows environment (MS Word, Outlook, Excel)
* Verbal and written communication skills
* Demonstrated problem solving skills
