Support and Service Coordinator

Wisconsin Community Services seeks a Support and Service Coordinator to connect adults, youth, and families to community-based behavioral health, housing, and justice support services. In this mission-driven, client-centered role, you will assess needs, develop service plans, coordinate referrals, and monitor progress while maintaining accurate documentation.
