Description

Job Purpose:

This position serves as case manager to individuals with intellectual/developmental disabilities, medical support needs, and/or traumatic and acquired brain injuries enrolled in Adult Day programming. The Case Manager II is responsible for maintaining the client record, communicating with stakeholders, and coordinating the overall care of the person served. The Case Manager II also promotes the importance of community integration and inclusion for all through education and service delivery.

This role indicates the ability to deliver a specialty service based on specific degree or certification, such as Art, Music, Speech, Occupational, Horticultural or Recreation Therapy.

  • Responsible for the maintenance of the client record. Completes all required documentation, in a timely manner, meeting best practice standards to uphold the integrity of the client record.
  • Maintains effective and supportive relationships with stakeholders, provides timely communication and coordinates and leads biannual plan of care conferences.
  • Provides a safe, therapeutic learning environment for all persons served. Creates new or supplemental components to existing curricula that are relevant, engaging, appeal to different learning styles and modalities, and meets the needs of the individuals served.
  • Responsible for assisting individuals at mealtime to ensure safety and promote a pleasant and respectful eating environment.
  • Conducts stand up meetings and mentors Direct Service Professionals and Program Support Professionals in classroom instruction, engaging persons served, adapting activities, and increasing level of comfort when running groups in the community. Demonstrates professionalism and encourages teamwork.
  • Identifies and introduces assistive technology, accommodations, compensatory strategies, or support plans as needed to improve client presentation and/or performance.
  • Represents MCFI and advocates for people with disabilities through formal and informal participation in professional and community organizations and interactions with community partners. Other duties as assigned
Qualifications

Required Education, Experience, Certifications, Licensure and Credentials: (Where appropriate, education and/or experience may be substituted) Minimum Required

Education:

Bachelor's degree in Community Counseling, Vocational Rehabilitation, Art, Music, Recreation, Speech or Occupational Therapy with noted experience in delivering these specialized services. Minimum

Required Experience:

Experience working in some capacity with individuals with intellectual and developmental disabilities or acquired and traumatic brain injuries. Required License – Certification – Registration:
Current First Aid certification or completed within first 90 days of hire. Current Non-Violent Crisis Intervention certification or completed within first 90 days of hire. Travel Type:
Up to 25% Required Valid Driver's License:
Valid Wisconsin driver's license Required Auto Insurance:
Vehicle liability insurance in accordance with Agency policy Knowledge, Skills, & Abilities:
Knowledge of case management principles and practices. Knowledge of community integration and inclusion best practices. Must have strong oral and written communication skills and demonstrate computer literacy. Ability to interpret observational data to make objective and impartial written/verbal reports. Must be comfortable conducting formal classroom sessions and apply various teaching methodologies to meet the needs of the people in programming.

Understanding of behavior modification techniques, compensatory strategies, and accommodations. Regular city and/or highway driving is required in agency vehicle. Use of public transportation may at times be needed as part of program curriculum.
