Advocate Health in Wisconsin seeks a full-time Social Worker for the Aurora Family Service - Supportive Counseling Services. The role includes psychosocial screening, care coordination, and education for patients and families.
You will work with a multidisciplinary team and provide counseling, case management, and crisis intervention. In-person at the Aurora Sinai Outpatient Center, Monday–Friday, with home visits as needed.
