Lutheran Social Services (LSS) seeks a Disability Services Consultant to support individuals with disabilities in accessing resources, housing, behavioral health, and community-based services. In this mission-driven, client-centered role, you will complete assessments, develop individualized service plans, and provide case management, advocacy, and education to promote independence and inclusion. You'll collaborate with interdisciplinary teams and community partners, connect clients to benefits and supports, and maintain accurate documentation while living out LSS's values of dignity, compassion, and social justice.

Responsibilities

  • Conduct intake assessments to identify client disability-related needs, strengths, and goals
  • Develop and coordinate individualized service and support plans for clients with disabilities
  • Connect clients to community resources, benefits, and accommodations to promote independence
  • Collaborate with interdisciplinary teams, families, and community partners to coordinate care
  • Provide advocacy, education, and guidance on disability rights and access to services
  • Maintain accurate, timely documentation and case records in compliance with agency standards
  • Monitor client progress, adjust service plans, and provide ongoing case management
  • Participate in team meetings, trainings, and quality improvement initiatives
  • Educate community stakeholders on inclusive practices and disability awareness
  • Align services with LSS's mission, values, and client-centered, trauma-informed practices
