Lutheran Social Services (LSS) seeks a Social Services Consultant to support individuals and families facing behavioral health, housing, and social challenges. In this role, you will assess client needs, develop individualized service plans, and connect clients to community resources, benefits, and LSS programs. You will provide advocacy, short-term counseling, and education while collaborating with an interdisciplinary team in a mission-driven, faith-based, and client-centered environment.

Responsibilities

  • Conduct assessments to identify client needs in behavioral health, housing, family support, and related social services
  • Develop individualized service and care plans aligned with LSS mission and program guidelines
  • Coordinate referrals and link clients to community resources, benefits, and support services
  • Provide short-term counseling, psychoeducation, and advocacy to help clients navigate complex systems
  • Collaborate with interdisciplinary teams, community partners, and faith-based organizations
  • Maintain accurate, timely documentation, case notes, and reports in accordance with standards
  • Monitor client progress, adjust service plans, and support goal achievement and self-sufficiency
  • Participate in case conferences, team meetings, and quality improvement initiatives
  • Educate clients and families on available programs, rights, and responsibilities
  • Uphold ethical, culturally responsive, and trauma-informed practice in all client interactions
