Wisconsin Community Services is seeking a Case Manager II to provide comprehensive, trauma-informed support to justice-involved individuals, youth, and families in Milwaukee. In this role, you will assess client needs, develop person-centered service plans, coordinate behavioral health, housing, and community resources, and advocate for clients across systems. You will maintain accurate documentation, monitor progress, and collaborate with multidisciplinary teams. Responsibilities
- Conduct comprehensive assessments of client needs, risks, and strengths
- Develop and implement person-centered service and care plans
- Coordinate behavioral health, housing, employment, and community resources
- Provide advocacy and support for justice-involved individuals, youth, and families
- Monitor client progress and adjust service plans as needed
- Maintain accurate, timely documentation and case notes
- Collaborate with multidisciplinary teams and community partners
- Utilize trauma-informed and equity-focused practices in all interactions
- Provide crisis intervention and safety planning as appropriate
- Participate in team meetings, training, and quality improvement efforts
