Role Description

The CCS Care Coordinator is a full-time, on-site role based in Milwaukee, WI, responsible for coordinating comprehensive community services and supports for individuals in the CCS program. This position involves conducting assessments, developing person-centered service plans, and collaborating with participants, families, and providers to ensure appropriate and timely care. The Care Coordinator monitors service delivery, adjusts plans as needs change, and advocates for participants to access medical, behavioral health, social, and community resources.

Daily tasks include maintaining accurate documentation, participating in interdisciplinary team meetings, and communicating regularly with stakeholders to promote continuity of care and positive outcomes. The role requires adherence to agency policies, regulatory standards, and trauma-informed, culturally responsive practice.

Qualifications
  • Strong care coordination and case management skills to support individuals with complex needs and connect them to appropriate services.
  • Effective communication and customer service skills to build trusting relationships with participants, families, and community partners.
  • Knowledge of home care and community-based support services, including the ability to navigate local resources and systems of care.
  • Experience working in social services, behavioral health, or related fields; familiarity with CCS or similar programs is beneficial.
  • Bachelor’s degree in social work, human services, psychology, or a related discipline, or equivalent combination of education and relevant experience.
  • Ability to maintain detailed, confidential records; strong organizational, time management, and problem-solving abilities.
  • Commitment to equity, inclusion, and non-discriminatory practices; capacity to work with diverse populations and respect varied cultures and identities.
  • Proficiency with basic computer applications and electronic health record or case management systems; valid driver’s license may be required for local travel.
