This is a
community-based, mobile position
whereyou’ll meet people where they are emotionally, physically, and withintheircommunities. You’ll work alongside individuals, families, providers, and multidisciplinary teams to ensure each person receives coordinated,person-centered support.

Pay range begins at $52,000

• Build meaningful relationships by meeting individuals where they are and creating trusting, respectful, and supportive connections.
• Coordinate individualized care by partnering with individuals, families, natural supports, service providers, and recovery teams to develop, implement, and update personalized recovery plans.
• Connect people to resources by identifying community services and completing and sending referrals for needed supports.
• Collaborate with providers including therapists,healthcare professionals, case managers, and other community partners topromote coordinated, high-quality care.
• Support recovery goals by monitoring progress toward self-identified goals and providing supportive coaching, psychosocial rehabilitation, and symptom-management support.
• Stay connected through regular in-person visits, phone calls, and follow-ups.
• Help remove barriers by ensuring individuals have access to transportation for program-related appointments.
• Advocate for individuals by helping them access timelyand appropriate services while empowering them to recognize and build ontheir strengths.
• Support individuals through challenges by responding to crises and setbacks with empathy, professionalism, andatrauma-informed approach.
• Document services accurately by maintaining timely and thorough documentation in accordance with agency, county, regulatory, and funding-source requirements.
• Staycurrent and compliant by completing all required county and agency trainings and following agency policies, procedures,Code of Conduct, and Core Values.

• Bachelor’sdegree in
  Social Work or a related field
  .
• Oneyear of experience working with individuals experiencing
  mental health, substance use, and/or co-occurring needs
  is preferred.
• Abilityto meet all WCS and contractor requirements, including criminal and caregiver background checks, driver’s record review, and drug screening.
• Valid driver’s license, reliable automobile, and sufficient insurance to meet agency requirements.

• Master’sdegree in
  Social Work or a related field
  .
• Experience working with
  Milwaukee County Behavioral Health Services
  .

• Astrong understanding of mental health, substance use, and/or cooccurringneeds.
• The ability to communicate clearly and provide calm, compassionate support during times of crisis.
• Experience with assessment, interviewing, recovery-oriented approaches, and harm reduction models.
• Apassion for
  mental health advocacy and person-centered care
  .
• Strong verbal and written communication skills and the ability to work effectively with diverse individuals, families, coworkers, supervisors,and community partners.
• The ability to work independently while also contributing to a respectful,collaborative team environment.
• Excellent organization, time management, and multitasking skills, with the abilityto meet deadlines and adapt to changing priorities.
