As a
Case Manager with our Community Support Program(CSP)and help adults living with serious and persistent mental illness and co-occurring substance use disorders build stability, independence,and meaningful lives in their community.

This is an opportunity to dopurpose-driven work while building your clinical career. You’ll receive hands-on experience, work alongside an Assertive Community Treatment (ACT) team, and have access toclinicalsupervision toward licensure.

Starting Pay: $44,000

As a CSP Case Manager, you’ll help participants move fromwhere they are today toward where they want to be.

• Completing ongoing assessments and developing person-centered treatment plans.
• Providing case management, supportive psychotherapy, symptom management, and crisis intervention within your scope of practice.
• Monitoringmental health symptoms and coordinating medication-related needs.
• Supportingparticipants in developing and achieving attainable recovery goals.
• Coordinatingmental health, healthcare, and substance use treatment services.
• Helpingparticipants access and maintain housing.
• Supportingmoney management and other activities of daily living.
• Helpingparticipants pursue employment and educational goals.
• Assisting with appointments, transportation, scheduling, and community resources.
• Conductingfrequent home visits and community-based outreach.
• Providing transportation to participants when needed, including driving an agencyvan in circumstances required by program policy.
• Working closely with participants’ families and natural supports.
• Participatingas an active member of an
  Assertive Community Treatment (ACT) team
  .
• Participating in group facilitation as assigned.
• Attending daily staffing, weekly team meetings, and in-service training.
• Maintaining accurate and timely clinical documentation in accordance with agency policy and DHS 63.
• Managingparticipant funds when appropriate.
• Collaborating with teammates to address both planned and unexpected participant needs.
