As a Hoarding Case Manager, you will guide clients through Catholic Charities' Hoarding Intervention and Treatment Program, the only program of its kind in the nation. The program helps individuals with indicators of hoarding disorder create safe, stable housing through case management, in-home coaching, therapy, and community collaboration. You will assess each client's needs, lead the development of their care plan with Home Coaches and Therapists, connect them to essential resources, and support them through the barriers that come with hoarding disorder.

• Assess Client Needs: Respond to client inquiries and conduct in-home assessments to understand each client's physical, social, and financial needs.
• Build Individualized Care Plans: Collaborate with Home Coaches and Therapists to develop a care plan for each client.
• Provide Hands-On Support: Work one-on-one with clients in their homes, helping with decluttering, organizing belongings, and other support in line with the care plan.
• Connect Clients to Resources: Help clients access essential services such as Medicare, food assistance, Family Care, and disability programs.
• Monitor Progress and Advocate: Evaluate treatment outcomes, advocate for additional resources, and provide personal support during crises.

$45,000-$47,000, depending on experience.

• Bachelor's degree in Social Work, Psychology, Sociology, or a related field required.
• At least six months of experience in a social service or health care setting required; 2+ years preferred.
• Prior experience working with older adults, individuals with disabilities, or people with cognitive, physical, or mental health challenges required.
• Valid driver's license, reliable transportation, and vehicle liability insurance of $100,000 per person / $300,000 per accident required.

As a Hoarding Case Manager, you will be a key advocate for your clients, helping them navigate services, overcome barriers, and work toward sustainable, long-term goals.
