* The Center for Veterans Issues (CVI) is seeking a compassionate and dedicated  
** Case Manager
** to join our Suicide Prevention Program. In this rewarding role, you will work directly with Veterans and their families, providing critical support, crisis intervention, care coordination, and advocacy to individuals at risk of suicide.

This position offers the opportunity to make a lasting impact by helping Veterans navigate challenges, access resources, and build pathways toward recovery, wellness, and stability.

** What You'll Do*
* ** Crisis Intervention & Safety Planning*
* + Conduct suicide risk assessments using the Columbia Suicide Severity Rating Scale (C-SSRS)

+ Develop individualized safety plans and Wellness Recovery Action Plans (WRAPs)

+ Provide crisis intervention and de-escalation support

+ Deliver suicide prevention education and gatekeeper training to Veterans, families, and community members

** Case Management & Care Coordination*
* + Conduct comprehensive assessments to identify participant needs, strengths, and barriers

+ Develop and monitor individualized service plans focused on recovery and wellness

+ Connect participants with VA and community resources, including:

+ Behavioral health services

+ Housing assistance

+ Employment resources

+ Benefits and supportive services

+ Assist Veterans with scheduling appointments and navigating systems of care

+ Advocate for participant rights and access to services

** Education & Coaching*
* Provide one-on-one and group education related to:

+ Suicide prevention and awareness

+ Recovery and resilience skills

+ Community resource utilization

+ Budgeting and financial literacy

+ Daily living and coping strategies

+ Understanding participant rights

** Community Outreach*
* + Build and maintain relationships with VA partners, Veteran service organizations, and community agencies

+ Participate in community events and outreach efforts

+ Expand referral networks to ensure Veterans receive comprehensive support

** Documentation & Compliance*
* + Maintain accurate and timely case documentation

+ Ensure compliance with agency policies and federal regulations

+ Participate in team meetings, trainings, and quality improvement initiatives

** What We're Looking For*
* ** Required Qualifications*
* + Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or a related field

** OR*
* + Associate degree plus 4-5 years of case management experience involving needs assessment and service planning

+ Minimum of two years of case management experience

+ Valid Wisconsin driver's license with a good driving record

+ Current vehicle insurance

+ Proficiency with Microsoft Office applications

** Preferred Qualifications*
* + Experience working with Veterans

+ Experience supporting individuals with mental health and/or substance use challenges

+ Familiarity with community resources and services within the region

+ Knowledge of Service Point or similar case management systems
