* Center for Veterans Issues (CVI) is seeking a compassionate, organized, and mission-driven
** Case Manager
** to join our Transitional Housing team. In this role, you will work directly with veterans experiencing homelessness, helping them access critical resources, overcome barriers, and achieve housing stability and long-term independence.

** Key Responsibilities*
* ** Intake & Assessment*
* + Conduct client intake assessments and determine program eligibility.
+ Assess veterans' housing, employment, health, mental health, and support needs.
+ Identify strengths, barriers, and service priorities.
** Individual Service Planning*
* + Develop and maintain individualized service plans (ISPs) with measurable goals.
+ Track progress toward housing, employment, wellness, and self-sufficiency objectives.
+ Review and update plans regularly in compliance with program requirements.
** Case Management & Resource Coordination*
* + Connect veterans with community resources and supportive services.
+ Coordinate referrals for:
+ Mental health services
+ Substance use treatment
+ Employment assistance
+ Education and training programs
+ Permanent housing opportunities
+ Medical and behavioral healthcare
+ Schedule appointments and assist with transportation coordination as needed.
** Crisis Intervention & Advocacy*
* + Provide crisis intervention and de-escalation support.
+ Assist residents in navigating healthcare, legal, and social service systems.
+ Advocate for access to benefits, services, and housing resources.
** Documentation & Compliance*
* + Maintain accurate and timely case notes and client records.
+ Enter documentation into HMIS and other case management systems.
+ Ensure compliance with agency policies, grant requirements, and confidentiality standards.
+ Monitor outcomes and maintain complete, audit-ready case files.
** Resident Support*
* + Provide ongoing encouragement, mentorship, and emotional support.
+ Educate residents on life skills, budgeting, wellness, job readiness, and independent living strategies.
+ Participate in case conferences, team meetings, and interdisciplinary collaboration efforts.
** Qualifications*

* Required:
*
* + Bachelor's degree in Social Work, Psychology, Human Services, or a related field.
+ Minimum of
** 2 years of case management experience** .
+ Valid driver's license with a good driving record.
+ Strong communication, organizational, and documentation skills.
+ Ability to manage multiple priorities in a fast-paced environment.
** Preferred:*
* + Master's degree in a related field.
+ 3-5 years of case management experience.
+ Experience working with homeless populations, veterans, or supportive housing programs.
+ Knowledge of veteran resources, housing systems, mental health, and substance use recovery services.
+ Experience using HMIS or electronic case management systems.
** Skills & Competencies*
* + Strong Microsoft 365 proficiency
+ Knowledge of Harm Reduction practices
+ Motivational Interviewing skills
+ Crisis intervention and conflict de-escalation
+ Excellent interpersonal and relationship-building abilities
+ Ability to work independently and as part of a collaborative team
** Working Conditions*
* + Primarily office-based environment
+ Local travel less than 10%
+ Standard business hours, 1st shift
+ Standing and walking up to 25% of the time
+ Occasional lifting of up to 15 pounds
