QA Nurse

As a QA Nurse at Gentiva, you will ensure the highest standards of clinical quality for home health and hospice patients. You will review charts, verify regulatory and documentation compliance, and identify improvement opportunities that protect patient safety and dignity. Working closely with field clinicians and leadership, you'll provide education, feedback, and support for corrective actions. Responsibilities

• Conduct clinical quality audits of home health and hospice patient records to ensure regulatory and Gentiva standards are met
• Review care plans, documentation, and medication records for accuracy, completeness, and timeliness
• Identify trends, gaps, and risks in clinical quality; recommend corrective actions and process improvements
• Provide feedback, coaching, and education to field clinicians on documentation and best practices
• Assist with incident reviews, root-cause analysis, and development of quality improvement initiatives
• Collaborate with nursing leadership, compliance, and interdisciplinary teams on quality metrics and outcomes
• Prepare quality reports and maintain accurate QA records for internal and external review
• Support accreditation, state, and federal survey readiness activities
• Participate in training, policy updates, and implementation of new clinical protocols
• Promote a culture of patient safety, dignity, and continuous improvement across care teams
