The Case Management Case Aide provides vital administrative and coordination support to case managers and social workers serving individuals in the Waiver Case Management (WCM) program across all counties. This role assists the case manager with administrative activities related to Waiver case management services. The Case Aide is integral in ensuring timely, accurate documentation, smooth client interactions, and effective communication within care teams.
This position handles tasks such as managing case files, scheduling appointments, and supporting documentation and billing processes. The incumbent complies with all applicable company policies and federal, state, and local regulations, including HIPAA, OSHA, and Minnesota Department of Human Services standards.
- Provide comprehensive administrative support to the Waiver Case Management (WCM) program across all assigned counties
- Assist in case preparation, organization, and file management, including filing screening materials and maintaining accurate, up-to-date documentation.
- Prepare and update service agreements, support plans, and other case documentation under the direction and oversight of the Case Management team.
- Schedule and coordinate client meetings, assessments, and care-related appointments.
- Perform data entry for screenings and service authorizations as assigned.
- Initiate provider and vendor contacts for service coordination (e.g., scheduling services, submitting referrals).
- Attend client appointments and support meetings on behalf of case managers as needed.
- Communicate with clients and families to support administrative needs (e.g., follow-ups, scheduling, information sharing).
- Collaborate with interdisciplinary teams and external providers to promote consistent, high-quality service delivery.
- Contribute to administrative projects, process improvements, and initiatives that enhance program operations.
- Perform additional responsibilities as assigned by leadership as the role evolves to meet program needs.
- Serve as a point of contact for clients and families for scheduling and information sharing, under management direction.
- Conduct non-clinical check-ins or intake assistance as requested by management.
- Provide communication and information assistance to support service coordination, always under the supervision of the case manager.
- Ensure culturally responsive and respectful communication in all client interactions.
- Perform data entry and maintain client records in accordance with DHS and agency standards.
- Track billing, authorizations, service timelines, and report deadlines.
- Audit and organize case files to ensure compliance and readiness for internal or external review.
- Ensure documentation meets DHS and HIPAA compliance requirements at all times.
- High school diploma or equivalent
- Have one year of experience as a case aide, administrative role, or in a closely related field or one year of post‑secondary education.
- Willingness to travel within the Metro Area, for applicable mileage reimbursements, with a current driver’s license, clean record, and personal vehicle with insurance; and able to transport clients in that vehicle.
- A valid Driver’s License, reliable vehicle, satisfactory driving record, and auto insurance are required.
- Must pass the required background check and remain eligible to provide services as defined by the Minnesota Department of Human Services.
