Overview
We are seeking a Detail‑Driven, Person‑Centered Designated Coordinator to support high‑quality service delivery for individuals receiving in‑home healthcare services. This role ensures that clients achieve their outcomes while maintaining strict compliance with 245D licensing requirements
.
The ideal candidate excels in communication, problem solving, documentation, and providing support to both clients and direct care staff. This is a highly impactful role that strengthens the connection between clients, caregivers, and the administrative team.

- Mileage reimbursement for client home visits

- Oversee, support, and evaluate daily service delivery to ensure clients are progressing toward their personal goals.
- Ensure all documentation, Support Plans, forms, and procedures meet 245D regulatory standards.
- Complete after‑visit documentation, maintain accurate records, and generate required reports.
- Communicate with case managers, counties, and care teams regarding follow‑up needs, requests, and service changes.
- Provide clear instruction, assistance, and competency observations for direct support staff.
- Act as the primary communication liaison and advocate for individuals receiving services.
- Review and complete incident reports, emergency reports, and corrective action items.
- Add new services, verify proper documentation and training, and support ongoing staff development.
- Conduct required 245D home visits and ensure compliance with annual paperwork, training, and service requirements.
- Bachelor's degree in human services, health, or education + 1 year of full‑time direct care experience with individuals with disabilities or seniors.
- Associate degree in human services, health, or education + 2 years of full‑time direct care experience.
- Postsecondary diploma in a related field + 3 years of full‑time direct care experience.
- 50 hours of human‑services‑related training + 4 years of full‑time direct care experience under qualifying supervision.
- Experience working in a 245D‑licensed setting
. - Experience providing services in client homes
. - Strong organizational, time‑management, and interpersonal communication skills.
