Overview

We are seeking a Detail‑Driven, Person‑Centered Designated Coordinator to support high‑quality service delivery for individuals receiving in‑home healthcare services. This role ensures that clients achieve their outcomes while maintaining strict compliance with 245D licensing requirements

• Oversee, support, and evaluate daily service delivery to ensure clients are progressing toward their personal goals.
• Ensure all documentation, Support Plans, forms, and procedures meet 245D regulatory standards.
• Complete after‑visit documentation, maintain accurate records, and generate required reports.
• Communicate with case managers, counties, and care teams regarding follow‑up needs, requests, and service changes.
• Provide clear instruction, assistance, and competency observations for direct support staff.
• Act as the primary communication liaison and advocate for individuals receiving services.
• Review and complete incident reports, emergency reports, and corrective action items.
• Add new services, verify proper documentation and training, and support ongoing staff development.
• Conduct required 245D home visits and ensure compliance with annual paperwork, training, and service requirements.

• Bachelor's degree in human services, health, or education + 1 year of full‑time direct care experience with individuals with disabilities or seniors.
• Associate degree in human services, health, or education + 2 years of full‑time direct care experience.
• Postsecondary diploma in a related field + 3 years of full‑time direct care experience.
• 50 hours of human‑services‑related training + 4 years of full‑time direct care experience under qualifying supervision.

• Experience working in a 245D‑licensed setting
