Job OverviewFairview is seeking a Clinical Oncology Pharmacist to support hematology, oncology, and bone marrow transplant (BMT) patients at the University of Minnesota Medical Center – East Bank. This hospital-based pharmacist role provides direct clinical pharmacy services in a complex, academic medical center, partnering closely with interdisciplinary care teams to ensure safe and effective medication therapy.
Schedule & Work DetailsHours: Full-time (80 hours bi-weekly)
Shift: Day/Evening
Weekend Rotation: Every third weekend
Practice Setting: Inpatient hospital / academic medical center
Primary Clinical Focus: Hematology, Oncology, and Bone Marrow Transplant (BMT)
- Provide clinical pharmacy services to oncology, hematology, and BMT patients, including medication review, monitoring, dose adjustment, and documentation in accordance with policies and regulations
- Serve as a clinical pharmacotherapy expert and consultative resource to physicians, nurses, students, residents, and interdisciplinary care team members
- Ensure safe, accurate, and compliant dispensing, sterile compounding, IV preparation, and distribution of medications
- Collaborate with care teams to optimize drug selection, formulary compliance, medication safety, and therapeutic outcomes
- Support patient and family education related to medication therapy
- Participate in quality improvement, regulatory compliance, and medication safety initiatives
- Maintain professional competency through continuing education, literature review, clinical meetings, and in-services
- Contribute to the education and training of pharmacy students and residents in an academic medical center environment
- Utilize pharmacy information systems and health IT tools to support daily operations and patient care
- Active Minnesota Pharmacist License
- Doctor of Pharmacy (Pharm
D) degree - Hospital or health‑system pharmacist experience
- Experience in oncology, hematology, or bone marrow transplant pharmacy
- Completion of an ASHP-accredited PGY1 residency program
- Experience practicing in a complex or academic healthcare environment
