Service Description

The Respite Care Service will be provided in the client’s home or a designated safe environment. This service is intended to offer temporary relief to the primary caregiver while ensuring the client receives appropriate supervision and support.

Objectives of Care
  • Provide companionship and supervision in a safe and supportive environment.
  • Support client’s independence while promoting emotional well-being.
  • Engage client in light recreational or leisure activities, as appropriate.
  • Ensure safety and monitor general well-being during service hours.
  • Provide caregiver with a consistent break to rest or attend to personal needs.
Client Needs/Support Areas
  • Assistance with getting off the bus upon return from her daily program.
  • Supervision to ensure safety.
  • Companionship and social engagement.
  • Redirection or assistance with simple daily routines (as needed).
  • Monitoring for health or emotional concerns and reporting to responsible party.
Expected Outcomes
  • Client remains safe and engaged during respite care hours.
  • Client receives timely and appropriate assistance upon arrival from daily program.
  • Primary caregiver experiences reduced stress and burnout.
  • Improved quality of life for both the client and caregiver.
Notes

All services to be documented daily, including activities performed and any observations.

Staff must follow client-specific care instructions and emergency protocols.

Any incidents or concerns must be reported immediately to the supervising agency or caregiver.
