Job Overview
The Medical Assistant Care Coordinator works with assigned patient population or individual patients to serve as the liaison between the patient, clinic and provider. The MACC acts to resolve patient problems that prevent them from receiving adequate and timely health care.
Key Responsibilities- Completes referrals for the team’s patients, including coordinating specialty appointments, communicating appointment information to the patient, tracking attendance, documenting missed appointments, and arranging follow‑up appointments.
- Supports clinic systems and processes that help standardize the patient experience; documents systems and processes and communicates them so that all staff provide a consistent experience.
- Provides RN care management support by sharing patient information as directed, maintaining registry data, and answering clinic staff questions regarding DBS data.
- Demonstrates the ability to provide care or service that reflects the needs of the population served, communicates respectfully, ensures a safe, secure environment, and partners with patient/caregiver to modify scheduling and communication based on patient needs.
- Performs miscellaneous duties as assigned, including filling in for the MA position and other duties that may arise.
- Participates in performance goals developed by manager, reports in the employee’s performance review, maintains core/universal competencies, completes all required new learnings, and demonstrates all critical competencies.
- Possess a Medical Assistant Diploma with proof of completion of a 160‑hour externship OR hold AAMA Certification OR hold AMT Registration.
