Shift Coordinator (Onsite)

Clifton Place serves adult men who are transitioning from incarceration and are under community supervision. Many residents have experienced long-term system involvement, trauma, substance use, housing instability, and disrupted access to healthcare and employment. The Clifton Place Shift Coordinator is actively engaged in the facility and building relationships with the residents who are stabilizing their lives, meeting court-ordered conditions, and building the skills, connections, and confidence needed to live safely and independently in the community.

This work requires consistency, boundaries, empathy, and a strong belief in people's capacity for change.

The Clifton Place Shift Coordinator holds responsibilities in three key areas: overseeing residents within the facility; managing residents outside the facility; and administrative and program-related tasks. The shifts are typically 8-hours, including evening or overnight schedules, with on-call duties and flexibility to cover open shifts when required.

The Shift Coordinator is a direct care role and the primary point of daily engagement with residents at Clifton Place. Shift Coordinators create safety, stability, and connection through consistent presence, relationship-building, supervision, and skill modeling. This role involves high levels of resident interaction, relationship building, milieu management, de-escalation, and daily living assistance, while supporting case management goals and maintaining accurate documentation.

Shift Coordinators do not provide clinical services and do not carry caseloads but play a vital role in supporting adult men who are transitioning from incarceration through day-to-day interactions.

This position does not have direct reports.

Duties and responsibilities include the following. Other duties may be assigned.

Supervision of Residents in the Facility:

  • Conduct hourly facility rounds to ensure the presence of all unsigned residents.
  • Monitor resident activities and uphold facility standards.
  • Establish communication with clients' professional contacts, including Correctional professionals, Mental Health, Chemical Health, and Medical, both initially and ongoing.
  • Maintain accurate and timely case notes in the Power 180 database for each resident and complete other programmatic forms.
  • Manage and provide direct services to clients, ensuring compliance with court-ordered conditions of release and lawful behavior.
  • Facilitate warrants and violation reports promptly.
  • Stay informed about the Department of Correction system, relevant laws, regulations, and policies.
  • Monitor and maintain public and facility safety within job assignments, including safety and security monitoring if assigned.
  • Demonstrate flexibility to work varying shifts, including evenings, holidays, weekends, and overnights, based on client and agency needs.

Supervision of Residents Away from the Facility:

  • Approve Sign-out cards & Daily Planners, clarifying resident plans and authorizing departures.
  • Conduct hourly resident status checks in the community, maintaining a Resident Check list and logging residents' current locations after reviewing phone logs and sign-out sheets.
  • Document all resident call-ins into the Power 180 database.
  • Facilitate warrants and violation reports promptly.

Administrative and Program Duties:

  • Generate Incident Reports for violations of sign-out, check-in, or accountability policies.
  • Coordinate program activities during shifts, such as meals and groups.
  • Enforce program rules and policies, documenting incidents as necessary.
  • Inspect rooms for cleanliness and orderliness.
  • Conduct random UAs or BAs as requested by Case Managers or as needed, complete UA processes accurately for submission to the lab.
  • Handle the completion of intakes.
  • Prepare, cook, and distribute meals for residents as assigned.

Resident Engagement, Milieu Management, and Relationship-Building:

  • Maintain active, consistent engagement with residents throughout the shift to support safety, connection, and positive peer interactions.
  • Create a calm, predictable, and supportive milieu through presence, modeling, and respectful communication.
  • Build trusting, professional relationships with residents through listening, encouragement, and mentorship.
  • Model healthy boundaries, communication, conflict resolution, and coping strategies.
  • Participate in recreational, educational, and group activities with residents as appropriate.
  • Actively participates in milieu management with treatment team collaboration.
  • Interact with residents whenever possible.

Safety and De-Escalation:

  • Maintain high situational awareness and intervene early to de-escalate conflict or distress using trauma-responsive approaches.
  • Conduct required safety checks, room checks, and bed checks, including overnight visual observations.
  • Respond to crises, emergencies, or safety concerns in accordance with training and program policy.
