People Incorporated Mental Health Services is seeking a Case Manager II to join the Targeted Case Management Metro Counties team. You will work with clients diagnosed with chronic mental illness, developing self-awareness and plans through frequent community meetings, while building long-term relationships and operating autonomously.
The role offers a hybrid schedule, with hours Monday–Friday 8–5. A $1,000 hiring bonus is available to external candidates. Salary range is $24.25–$25.75 per hour.
