Compensation: $65,000-$85,000 per year , depending on experience

What You'll Do
  • Complete comprehensive psychosocial assessments for hospice patients and families.
  • Provide emotional support, counseling, crisis intervention, and grief support throughout the hospice journey.
  • Help families understand available resources, community services, financial assistance, and advance care planning.
  • Advocate for patients while ensuring their emotional, cultural, and psychosocial needs are respected.
  • Collaborate closely with nurses, physicians, chaplains, hospice aides, and the interdisciplinary team to provide holistic care.
  • Support family meetings, care planning, discharge coordination when appropriate, and bereavement planning.
  • Maintain timely, accurate documentation while ensuring compliance with hospice regulations and Medicare Conditions of Participation.
  • Be a trusted source of guidance, reassurance, and compassion during life's most meaningful moments.
What We're Looking For
  • Bachelor's or Master's Degree in Social Work (BSW or MSW).
  • Hospice experience is required.
  • Strong understanding of grief, loss, end-of-life care, and family dynamics.
  • Exceptional communication, active listening, and relationship-building skills.
  • Ability to work independently while thriving as part of an interdisciplinary team.
  • Compassionate, dependable, organized, and emotionally resilient.
  • Passion for serving patients and families with dignity, empathy, and respect.
