Town & Home Health Aide; HHA)

Location: Minneota, MN Type: Full-Time & Part-Time Schedule: Days

Starting Pay $17 - $19.50 / hour

Provide resident care and delegated nursing tasks under the supervision of the licensed nurse, including medication passing, personal care, and person-centered support to clients in the community. HHAs are required to travel to clients' residences and deliver services delegated by the RN supervisor.

• Travel to clients' residences to deliver care delegated by the RN supervisor.
• Carry out the plan of care developed by the RN, based on each client's physical, psychosocial, cultural, and spiritual needs.
• Provide direct client care and report condition and changes to the nurse.
• Know each client as a whole person, including likes and dislikes, and bring that into the care provided (person-centered care).
• Provide or assist with bathing, dressing, and grooming per the RN's plan of care.
• Manage time well and prioritize tasks across visits.
• Document client care within established policies and scope of practice.

• Passes all competency testing, including medication administration
• Good communication skills; able to read, write, and communicate in English
• Reliable personal vehicle to travel between client residences; mileage is tracked
• Successful Minnesota DHS background study

Strong organizational skills, the ability to observe and report resident status and carry out the plan of care, and the ability to manage multiple priorities and stay calm in an emergency.

• Previous assisted living or home care experience
• CNA or HHA certified
