Overview
We are seeking an experienced Nurse Practitioner to join our mobile primary care team serving SNFs, LTCs, and ALFs in Minnesota.
Responsibilities- Provide comprehensive primary care, manage acute and chronic conditions, and build strong relationships with residents, families, and care teams.
- Perform assessments, order diagnostics, develop treatment plans, and prescribe medications within guidelines.
- Collaborate with facility staff to ensure seamless, patient-centered care and support quality improvement initiatives.
- MN NP license
- Board certification
- Preferred experience in geriatric or post-acute settings
