Terrace at Crystal - The Licensed Practical Nurse (LPN) at The Terrace at Crystal provides direct nursing care to residents under the supervision of a Registered Nurse (RN) or physician. The LPN assists in implementing care plans, administering medications and treatments, supervising nursing assistants, and ensuring resident safety, dignity, and quality of care in compliance with federal, state, and facility regulations.
Essential Duties and Responsibilities Resident Care- Provide direct nursing care to residents in accordance with physician orders, care plans, and facility policies.
- Administer medications and treatments accurately and safely per state scope of practice.
- Monitor residents’ physical and emotional conditions and promptly report changes to the RN or physician.
- Assist with admissions, discharges, and transfers as assigned.
- Support resident rights, dignity, privacy, and person-centered care.
- Supervise and provide direction to Certified Nursing Assistants (CNAs) during assigned shifts.
- Ensure delegated tasks are completed safely and timely.
- Communicate effectively with interdisciplinary team members regarding resident care needs.
- Accurately document nursing care, treatments, and observations in the electronic medical record (EMR).
- Participate in shift reports, care conferences, and interdisciplinary meetings as assigned.
- Maintain confidentiality in accordance with HIPAA and facility policy.
- Follow infection prevention and control policies, including hand hygiene, PPE use, and isolation precautions.
- Promote a safe environment by identifying and reporting hazards or unsafe conditions.
- Adhere to all CMS regulations, Minnesota Department of Health requirements, and The Terrace at Crystal policies.
- Participate in Quality Assurance and Performance Improvement (QAPI) initiatives.
- Demonstrate regulatory awareness and professionalism during state surveys and audits.
- Assist with resident transport to activities, therapy, or appointments as assigned.
- Perform other duties within the LPN scope of practice as assigned.
- Current, active Licensed Practical Nurse (LPN) license in the State of Minnesota.
- Ability to work collaboratively with residents, families, and the care team.
- Strong communication, organizational, and clinical skills.
- Ability to follow written and verbal instructions.
- Successful completion of background checks, health screenings, and TB testing per state and facility requirements.
- Previous experience in a skilled nursing, assisted living, or long-term care setting.
- Experience with electronic medical record (EMR/EHR) systems.
- Knowledge of CMS regulations and resident rights.
- Ability to lift, push, pull, and transfer residents with or without assistance.
- Ability to stand, walk, bend, stoop, kneel, and reach for extended periods.
- Ability to respond quickly and appropriately to emergency situations.
- Long-term care / skilled nursing or assisted living environment.
- Exposure to residents with complex medical, behavioral, or cognitive needs.
- Possible exposure to infectious diseases.
- Compliance with all federal, state, and facility policies and procedures.
- Timely completion of assigned duties and documentation.
- Attendance, punctuality, and professional conduct per facility standards.
- Participation in required training, education, and competency evaluations.
