In-Home Health - Nurse Practitioner or Physician Assistant Minneapolis, MN

Signify Health, part of CVS Health®, is seeking a Full‑time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation and, if ordered by the health plan, additional diagnostic tests such as diabetic eye exams or spirometry. You will use an iPad and an intuitive clinical workflow for each evaluation.

You are not required to prescribe medicine, order lab tests or alter people’s current treatment regimen.

• Build trusted relationships to improve health outcomes
• Conduct in‑home visits to provide personalized care to members across the nation
• Fulfill role requirements and collaborate with Signify Health clinicians
• Travel locally and within licensed states as needed
• Conduct virtual visits when required
• Provide all Signify Health services, including Diagnostic & Preventive Services such as spirometry and DEE

• Active, unrestricted license(s) in the coverage area(s) (Minnesota only)
• Completion of 2,080 hours of post‑graduate collaborative practice in an integrated clinical care setting
• Board certification (required)
• Multi‑state licenses (preferred)

Anticipated weekly hours: 40 hours

Pay range: $87,035.00 – $ (base hourly rate or annual salary, depending on experience, education, geography, and other factors)
