Job Title: Hospice Admissions RN – Be the First Voice Families Trust (Registered Nurse)

Location: Greater Twin Cities Metro Area, 21-county service area (territories assigned based on coverage needs)
Schedule: Full-Time or Part-Time with 1 on-call shift per week (backup) and every 4th-7th weekend

- Travel to patient homes, assisted living, and care facilities to complete hospice admissions.
- Perform clinical eligibility assessments and initiate services
. - Lead goals-of-care conversations with patients and families.
- Provide clear education on hospice philosophy, services, and expectations
. - Coordinate with intake, physicians, and the interdisciplinary team.
- Ensure a smooth, confident transition into hospice care
. - Maintain accurate and timely documentation

- Experience in hospice, oncology, palliative care, or high-acuity hospital settings
. - Strong ability to communicate clearly and build trust quickly
. - Confident clinical judgment and decision-making skills.
- Comfort working independently in a field-based role
. - Reliable transportation and willingness to travel.
