Responsibilities- Making scheduled and as‑needed visits to patient homes and facilities
- Managing symptoms, monitoring health changes, and offering wound care
- Supporting emotional needs and educating family members
- Documenting clearly in our EMR system
- Collaborating with a dedicated interdisciplinary team
- Experience in hospice preferred (palliative, oncology, or med‑surg with EOL experience also welcome)
- Confidence in working independently and managing time well
- Strong interpersonal and documentation skills
- Driver’s license and dependable transportation
- A startup energy with strong clinical infrastructure
- Flexible scheduling and supportive leadership
