Position: Nurse Practitioner (PRN) - In-Home Health Assessments

Overview

$1,500 Sign‑On Bonus

Responsibilities

During a visit of up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment, and other risks to provide comprehensive insight that is difficult to capture in routine office visits. The Matrix Comprehensive Health Assessment (CHA) helps improve quality of care and close multiple care gaps with a single visit.

Key duties include:

  • Conduct adult/geriatric assessments – medical history, diagnosis, treatment, health education, physician referrals, case‑management referrals, follow‑up, and clear documentation per Matrix guidelines and protocols.
  • Administer and order point‑of‑care and lab‑based testing as needed.
  • Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
  • Work with Primary Care Physicians on patient education and follow‑up.
  • Provide services at members’ places of residence, including homes and Skilled Nursing Facilities.

Income based on number of assessments completed; earning potential ranges from $1,800 – $11,500 per month (per‑assessment pay $90 – $115 depending on state).

Qualifications

• Master’s degree or equivalent experience and satisfactory completion of NP licensure

• Active NP licensure in the state of practice, including prescription authority or ability to obtain it

• Board certification in one of:
Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care (AANP, AACN, or ANCC)

• Current BLS, ACLS, or CPR certification

• 1 year NP experience preferred; new grads encouraged

• Valid state driver’s license and automobile insurance for the state of residence

• Bodily injury coverage: $100,000 per person / $300,000 per incident;
Property damage $50,000;
Collision/comprehensive deductible $1,000 or less

• Strong computer skills and familiarity with employee health/medical record software

• Excellent verbal and written communication skills
