Overview

The Nurse Practitioner provides personal, direct primary care to patients with acute and/or chronic illnesses in collaboration with physicians. The role assesses individuals and families using history, physical exam, and appropriate diagnostic tests/x-rays. This position formulates, implements, and evaluates patient care management according to practice guidelines, assisting individuals and families in mobilizing strengths in dealing with health maintenance, self‑care skills, adaptation to illness, health promotion, death, grief, and bereavement.

The position is .79 FTE, working 31.6 hours per week, including Saturday (8am-12pm) Walk‑In Clinic rotation.

Location:

West Central Minnesota, Renville County.

Responsibilities
  • Assess patient to determine physical and emotional needs.
  • Obtain complete medical history and physical data on patients.
  • Order appropriate diagnostic and therapeutic procedures as needed.
  • Perform a complete physical exam and record findings.
  • Collaborate with physicians in managing acute and long‑term medical needs of patients.
  • Synthesize data to determine preliminary diagnosis and therapeutic plan utilizing principles of prevention.
  • Prescribe and administer medications and injections according to physician directions.
  • Maintain and review patient records, charts, and other pertinent information.
  • Post tests and examination results.
  • Perform procedures approved within scope of practice and as instructed by physician.
  • Delegate care and make appropriate referrals to specialties.
  • Respect physical privacy of patients.
  • Interview and advise patients regarding health and illness prevention.
  • Provide patient education.
  • Instruct patients regarding medication and treatment instructions.
  • Establish patient‑specific goals that are consistent with the overall plan of care.
  • Network with health team members by communicating data based on diagnosis to provide continuity of care.
  • Recommend community resources to meet patient and family needs.
  • Communicate changes in client status with provider and/or family members.
  • Modify, implement, and evaluate individualized teaching plan in order to restore, maintain or promote health.
  • Collaborate with other health team members in intra/inter departmental issue recognition and resolution.
  • Give pertinent and timely report to co‑workers.
  • Utilize electronic medical record effectively.
  • Document accurately and legibly.
Qualifications
  • Graduate of an accredited Physician Nurse Practitioner program.
  • MN APRN license and certification as a Family Nurse Practitioner by a national agency recognized by the MN Board of Nursing.
  • Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS) or Advanced Trauma Life Support (ATLS) or Cardiac Advanced Life Support (CALS).
  • Drug Enforcement Administration Registration.
