Responsibilities
  • Provide individual assistance to patients, families, and caregivers to overcome healthcare system barriers and provide timely access to quality healthcare.
  • Utilize the nursing process to provide patient education and access to relevant internal and external resources.
  • Facilitate informed decision‑making and timely access to high quality health and psychosocial care through all phases of the care continuum.
  • Collaborate routinely with internal departments and external referral resources, including scheduling, financial counseling, lab, pharmacy, social work, nursing, surgical, radiation and imaging, dietary, and others.
  • Interface frequently with practice‑based leadership teams, physicians, and clinical staff to promote and engage the patient navigation process.
  • Coordinate care during the full continuum of the patient experience.
  • Identify potential and real gaps in care delivery; seek out opportunities for avoidance and/or closure.
  • Prepare for, organize, and lead the Patient Care Conference (huddle) at regular intervals; communicate interventional outcomes to the full care team in a timely manner.
  • Perform assessment of patient risk stratification.
  • Perform basic psychosocial assessments and provide emotional support; delegate and/or make referrals for additional screening and support as needed.
  • Develop appropriate nursing diagnoses and corresponding action plans as needed.
  • Support and/or provide education, adherence, and side‑effect monitoring.
  • Participate in survivorship and advance care planning visits; make referrals as needed.
  • Participate in research, quality, and performance improvement, including reporting on navigation metrics.
  • Support patient advocacy and empowerment.
  • Be knowledgeable of practice operations guidelines, practice organization, and healthcare economics.
Qualifications
  • Graduate from an accredited program for nursing education required (BSN preferred).
  • Current licensure as a registered nurse in the state of practice.
  • Nurse Navigator Certification preferred.
  • Current BLCS or ACLS certification required.
Business Experience
  • Minimum five (5) years of nursing experience required.
  • Basic knowledge of local community resources.
  • Colorectal experience preferred.

Physical Requirements

Large percent of time performing computer‑based work is required. Requires standing and walking for extensive periods of time. Occasionally lifts and carries items weighing up to 40 lbs. Requires corrected vision and hearing to normal range.
