SUMMARYHennepin Healthcare has a full-time (1.0 FTE) opening for a Nurse Practitioner or experienced Physician Assistant in our Family & Community Medicine Department – supporting our community clinics within Hennepin County. The current position is at our Golden Valley Clinic, but future needs could warrant working at additional clinic locations.
This position will work days M–F at this time.
RESPONSIBILITIESEvaluation, diagnosis and treatment of patients with adverse health conditions, and providing education to patients and families within scope of practice and in collaboration with the supervising/collaborating physician.
Provide direct primary and acute care services to adult and/or pediatric patients as part of the primary care inter-disciplinary team in the ambulatory department
Perform initial and on-going assessment of patients’ medical, physical and psychosocial status
Obtain and evaluate health and medical history; perform physical exam, conduct preventive screening procedures based on age and history
Identify medical and health risks
Complete accurate and timely documentation of observations, interventions, plan of care, referrals and follow-up Formulate appropriate differential diagnosis based on history, physical examination and diagnostic testing
Identify needs of the individual, family or community and develop a plan of care and/or education plan Order, conduct, and interpret diagnostic tests, therapeutic treatments; may prescribe, administer and dispense therapeutic devices and legend drugs
Make appropriate referrals to other health professionals and community agencies
Reassess and modify the plan as necessary to achieve medical and health goals
For the department of pediatrics performs evaluations, including pertinent history and physical exam on patients assigned to include normal newborn babies in the nursery
Contribute to on-going medical and nursing staff teaching programs
Develop a patient education plan relevant to the condition and patient/ family needs
Participate in performance improvement activities as assigned
Adhere to productivity standards established by the department
Assess need for personal growth and development and take the initiative in seeking opportunities to meet this need Foster a service that is consistent with the family-centered care and shared management philosophy of the department
Demonstrate respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment
Promote effective working relations and work effectively as part of a department/ unit team inter-departmentally to facilitate the department’s/ unit’s ability to meet its goals and objectives
Perform other related duties but only after appropriate training QUALIFICATIONS
Minimum Qualifications:
Master's degree in Nursing or in Physician Assistant studies from an accredited program
Current license to practice as an Advanced Practice Registered Nurse (APRN) or Physician Assistant in the State of Minnesota (or eligibility)
NP candidates must have current certification as a Family Nurse Practitioner or ability to obtain within 2 months of offer
