Organizational Responsibilities- Adhere to St. Francis Health Services’ mission and core values while performing company business.
- Maintain confidentiality of company and resident‑related information at all times.
- Serve on, participate in, and attend various committees as appointed by the Director of Nursing or Administrator.
- Implement and promote quality improvement recommendations.
- Perform administrative requirements, such as completing necessary forms, reports, and submitting them to the Director of Nursing or Administrator.
- Identify specific standards that contribute to rating.
- Do not disclose resident’s protected health information; promptly report suspected or known violations to the Administrator.
- Provide care in privacy and knock before entering a resident’s room or care area such as tub rooms.
- Report grievances and complaints from residents, families, or visitors following established procedures.
- Provide direct resident care and monitoring, making prompt initial contact upon admission or return from hospital.
- Obtain history, observe resident’s physical and emotional condition, and report issues to appropriate personnel while documenting findings.
- Delegate tasks to trained medication aides (TMAs) and nursing assistants as appropriate.
- Follow and recommend updates to the plan of care, including immediate and long‑term goals.
- Keep up‑to‑date knowledge of each resident’s care plan and inform nursing assistants of objectives and goals.
- Monitor nursing care to assure compliance with the care plan and report significant changes to the Nursing Supervisor.
- Perform basic nursing procedures safely, maintain technical skills, and perform lab draws as needed.
- Ensure medications are safely administered, practicing the eight rights of medication administration.
- Accurately chart medication administration and observe resident response, reporting errors as required.
- Follow drug‑control procedures and maintain medication carts, including narcotics counting and state/federal regulations.
- Maintain a complete clinical record of each resident’s stay, including vital signs, assessments, and documentation of changes in condition.
- Monitor resident needs and implement appropriate interventions, notifying physicians, DON, Administrator, and other department heads as necessary.
- Take direct action in emergencies, recognize potential emergencies, and notify appropriate leadership.
- Communicate clearly and appropriately with physicians, families, nursing assistants, and staff, ensuring confidentiality and HIPAA compliance.
