Our care teams travel directly to patients who reside in Assisted Living, Memory Care and Group Home communities throughout Minnesota, Wisconsin and Florida and are supported by clinical operations and administrative colleagues who work remotely or at our corporate offices in Stillwater, Minnesota, and Tampa, Florida.

Position Overview

We are seeking a full-time experienced Psychiatric Mental Health Nurse Practitioner to join our growing team. In this role, you will provide comprehensive psychiatric evaluation and management, support residents with complex behavioral health needs, and collaborate with interdisciplinary care teams to deliver integrated, person-centered care. This position offers autonomy in clinical practice, a supportive team environment, flexible scheduling, and consistent patient panels.

Key Responsibilities

As a vital member of our clinical team, your day-to‑day will involve high‑quality, compassionate care tailored to our residents' unique journeys.

  • Comprehensive Evaluations: Conduct psychiatric evaluations, mental status examinations, and behavioral assessments—primarily via telehealth, virtual visits.
  • Treatment Planning: Recommend adjustments through the case review process or in ad hoc consultations, including medication recommendations and other interventions. Advise on treatment for patients who may need more intensive or more specialized mental health care, supporting treatment in the medical setting until patients can be engaged in specialized care as appropriate.
  • Medication Management: Monitor, adjust, and manage psychotropic medications, ensuring safety and efficacy.
  • Interdisciplinary

    Collaboration:

    Work in collaboration with primary care providers, therapists, social workers, and nursing staff to ensure a seamless, holistic approach to patient wellness.
  • Complex Care Support: Provide targeted interventions and crisis management support for residents with complex, co‑occurring behavioral health needs.
  • Documentation: Maintain precise, timely documentation within the Electronic Health Record (EHR) system in compliance with regulatory standards.
Qualifications & Requirements
  • Graduate of an accredited Nurse Practitioner or Physician Assistant Education Program
  • Board Certified through ANCC
  • Licensed or ability to obtain licensure(s) through the Board of Nursing
  • Ability to obtain and/or maintain DEA certification for the State(s) of required of work
  • Proven ability to communicate effectively with strong verbal skills
  • Excellent interpersonal and customer service skills
  • Must have a valid and unrestricted driver’s license and minimum state‑required liability auto insurance; local travel is required

Preferred Qualifications:

  • 2+ years of clinical experience as a Psychiatrics Mental Health Nurse Practitioner
  • Compensation: $140k - 180k
