Registered Nurse (RN) Care Coordinator – Senior Living
Location:
Spokane, WA (Multiple Senior Living Facilities)
Employment Type:
Contract (4–6 months, FMLA coverage – maternity leave)
Schedule:
Monday–Friday, 8:00 AM – 5:00 PM
Travel:
Required within local facilities during shift
We are seeking a compassionate and detail-oriented Registered Nurse (RN) Care Coordinator to support Senior Living populations during a temporary maternity leave coverage assignment.
In this role, you will collaborate closely with providers, advanced practice clinicians (APCs), and interdisciplinary care teams to ensure high-quality, coordinated care for residents. You will play a key role in care coordination, clinical documentation, patient assessment, and care management, particularly for patients with complex or chronic conditions.

- are Assist in managing patients during acute episodes or chronic condition exacerbations
- Support care delivery during short-term sub-acute rehabilitation stays
- Coordinate therapy services and interdisciplinary care plans
- Ensure accurate, complete, and timely clinical documentation
- Assist providers with documentation, coding, and medication reconciliation
- Maintain compliance with quality measures and regulatory standards
- Record verbal orders from providers in accordance with RN scope of practice and facility policies
- Perform physical assessments and monitor changes in patient condition
- Communicate updates to providers and care team members promptly
- Participate in nursing rounds and ongoing patient evaluations
Participate in interdisciplinary meetings, case conferences, and facility-based discussions. Schedule and attend family meetings and care plan conferences. Partner with care teams to improve patient outcomes and care experiences.
Training & Quality Initiatives- Support training coordination, tracking, and compliance initiatives.

- Current Basic Life Support (BLS) certification
- Ability to perform physical assessments and meet physical requirements (including lifting up to 30 lbs)
- Access to reliable transportation for travel between facilities
- Fully vaccinated for COVID-19 (as required by client and regulatory guidelines)
- Ability to pass a pre-employment drug screening
- 1+ year of clinical experience (long-term care or senior living preferred)
- Experience managing patients with complex medical, behavioral, or social needs Knowledge of geriatrics and chronic illness care management.
- Familiarity with electronic medical records (EMR) systems.
- Strong organizational, communication, and time management skills. Ability to work effectively across diverse care team
