Syncare Memory Suites is seeking a compassionate, experienced, and highly organized Licensed Practical Nurse (LPN) Clinical Manager to lead daily clinical operations and help deliver exceptional memory care services.

This full-time, on-site leadership role is based in Plymouth, Minnesota, and is ideal for an LPN who is passionate about dementia care, resident advocacy, and team development. The Clinical Manager serves as a key member of our leadership team, overseeing resident health services, supervising caregivers, ensuring regulatory compliance, and fostering a culture of excellence in person-centered care.

• Oversee and coordinate daily clinical operations within the memory care residence.
• Conduct resident assessments and monitor changes in health status.
• Administer medications and treatments in accordance with physician orders and nursing standards.
• Develop, implement, and update individualized care plans.
• Collaborate with physicians, pharmacists, therapists, and other healthcare professionals to ensure optimal resident outcomes.
• Respond appropriately to urgent clinical situations and resident care needs.
• Supervise, schedule, and support caregiving staff.
• Provide coaching, mentorship, and performance feedback.
• Promote a positive, collaborative, and resident-focused culture.
• Support onboarding and ongoing staff education, particularly in memory care best practices and dementia-specific approaches.

• Ensure compliance with Minnesota assisted living regulations and company policies.
• Maintain accurate and timely clinical documentation.
• Monitor infection prevention and control practices.
• Participate in quality improvement initiatives and clinical audits.
• Support survey readiness and continuous regulatory compliance.

• Build trusting relationships with residents and family members.
• Provide education, updates, and emotional support to families navigating dementia care.
• Serve as a clinical resource and advocate for residents' well-being.

• Active Licensed Practical Nurse (LPN) license in the State of Minnesota.
• Minimum of 2 years of nursing experience in assisted living, long-term care, memory care, or a related senior care setting.
• Strong clinical assessment, medication administration, and care coordination skills.
• Experience supporting individuals living with Alzheimer’s disease or other forms of dementia.
• Knowledge of Minnesota assisted living regulations and healthcare documentation requirements.
• Current CPR certification.
• Excellent communication, interpersonal, and organizational skills.
• Ability to effectively lead and motivate care teams while maintaining a compassionate, resident-centered approach.

• Previous supervisory or clinical management experience.
