Position SummarySyncare Memory Suites is seeking a compassionate, experienced, and highly organized Licensed Practical Nurse (LPN) Clinical Manager to lead daily clinical operations and help deliver exceptional memory care services.
This full-time, on-site leadership role is based in Plymouth, Minnesota, and is ideal for an LPN who is passionate about dementia care, resident advocacy, and team development. The Clinical Manager serves as a key member of our leadership team, overseeing resident health services, supervising caregivers, ensuring regulatory compliance, and fostering a culture of excellence in person-centered care.

KEY RESPONSIBILITIES Clinical Leadership & Resident Care- Oversee and coordinate daily clinical operations within the memory care residence.
- Conduct resident assessments and monitor changes in health status.
- Administer medications and treatments in accordance with physician orders and nursing standards.
- Develop, implement, and update individualized care plans.
- Collaborate with physicians, pharmacists, therapists, and other healthcare professionals to ensure optimal resident outcomes.
- Respond appropriately to urgent clinical situations and resident care needs.
- Supervise, schedule, and support caregiving staff.
- Provide coaching, mentorship, and performance feedback.
- Promote a positive, collaborative, and resident-focused culture.
- Support onboarding and ongoing staff education, particularly in memory care best practices and dementia-specific approaches.
- Ensure compliance with Minnesota assisted living regulations and company policies.
- Maintain accurate and timely clinical documentation.
- Monitor infection prevention and control practices.
- Participate in quality improvement initiatives and clinical audits.
- Support survey readiness and continuous regulatory compliance.
- Build trusting relationships with residents and family members.
- Provide education, updates, and emotional support to families navigating dementia care.
- Serve as a clinical resource and advocate for residents' well-being.
- Active Licensed Practical Nurse (LPN) license in the State of Minnesota.
- Minimum of 2 years of nursing experience in assisted living, long-term care, memory care, or a related senior care setting.
- Strong clinical assessment, medication administration, and care coordination skills.
- Experience supporting individuals living with Alzheimer’s disease or other forms of dementia.
- Knowledge of Minnesota assisted living regulations and healthcare documentation requirements.
- Current CPR certification.
- Excellent communication, interpersonal, and organizational skills.
- Ability to effectively lead and motivate care teams while maintaining a compassionate, resident-centered approach.
- Previous supervisory or clinical management experience.
