Travel Nurse RN – Home Health (Plymouth, MN)

Start Date:

ASAP. Duration: 13 weeks.

Shift: 8 hours per day, 5 days a week (40 hours per week).

Employment Type:

Travel Nursing.

• Provide patient care in a home health environment, including assessment, plan implementation, and evaluation of nursing care for home care patients.
• Coordinate care with interdisciplinary team and family members, ensuring quality and continuity of care.
• Document nursing care accurately and timely, ensuring compliance with regulatory and client documentation standards.
• Adhere to infection control, safety, and patient confidentiality policies.

• Registered Nurse (RN) licensure in the state of Minnesota (or able to obtain within 45 days).
• Minimum of 1 year of home health nursing experience preferred but not mandatory.
• Strong clinical assessment and patient education skills.
• Excellent communication, organizational, and time‑management abilities.
