Nurse Practitioner, In-Home Visits

Overview

Signify Health, part of CVS Health®, is seeking a Part‑time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations. The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and, if ordered, additional diagnostic tests (e.g., diabetic eye exams, spirometry). The clinician will connect one‑on‑one with patients in their homes and assist with navigating the healthcare system.

• Conduct in‑home visits using an iPad and an intuitive clinical workflow.
• Provide all Signify Health services as indicated, including diagnostic and preventive service products (spirometry, DEE, etc.).
• Join the national network of clinicians dedicated to improving health outcomes.
• Visit patients nationwide, aiming to connect individuals to appropriate care.
• Maintain flexibility to travel locally and within licensed states as necessary.
• Conduct virtual visits as needed.

Clinicians work in the private residences of health‑plan members. Environments may include household pets, secondhand smoke, and other potential airborne allergens or irritants. Candidates must be able to perform duties safely in such settings.

• Active, unrestricted license(s) in the coverage area(s).
• Completion of 2,080 hours of post‑graduate collaborative practice hours as an NP or PA in an integrated clinical care setting (required).
• Board certification (required).
• Multi‑state licenses (preferred).
• Completion of the appropriate attestation with the state of Minnesota.

The typical pay range for this role is $41.85 – $90.13 per hour.
