Job Title: RN Visit Nurse – Home Health, 1:1 Patient Care | $4,000 Sign-On Bonus

Compensation: $75,000 to $100,000 per year plus a $4,000 Sign-On Bonus

Every visit gives you the opportunity to improve someone's quality of life.
You'll care for patients recovering from illness, surgery, injury, or chronic medical conditions while helping them remain independent in the comfort of their own homes.
Rather than seeing patients for only a few minutes, you'll have the opportunity to provide thoughtful, individualized care while becoming a trusted member of each patient's healthcare team.
What You'll Be Doing- Provide skilled home health nursing visits throughout your assigned territory
- Complete comprehensive patient assessments
- Perform wound care
, medication management, injections, and other skilled nursing services - Monitor patient progress and communicate changes to physicians
- Educate patients and caregivers on disease management and treatment plans
- Coordinate care with therapists, social workers, and interdisciplinary team members
- Complete timely documentation within our Homecare Homebase (HCHB) EMR
- Help reduce hospitalizations through proactive, high-quality patient care
- Active Minnesota Registered Nurse (RN) license
- Previous nursing experience in home health, hospital, rehabilitation, skilled nursing, or clinical care
- Strong assessment and critical thinking skills
- Ability to work independently while collaborating with an interdisciplinary team
- Excellent communication and organizational skills
- Reliable transportation and willingness to travel throughout the local service area
