Registered Nurse - Home Health - Minneapolis, MN

Your next nursing opportunity starts with a full-time, 13-week home health contract for an experienced Registered Nurse (RN) with OASIS proficiency. Schedule:

• Full-time, Monday through Friday
• On-call responsibilities may be required
• Daily visit objectives to be met

Qualifications:

• Active RN license
• At least 1 year of recent home health RN experience
• Strong working knowledge of OASIS documentation (mandatory)
• Preferably proficient with Home Care Home Base software
• Capable of managing a structured, full-time caseload and meeting daily visit goals
• Reliable transportation and valid drivers license are essential

Key Responsibilities:

• Deliver in-home nursing care and skilled clinical interventions
• Perform comprehensive patient assessments and complete OASIS documentation timely and accurately
• Collaborate effectively with physicians and interdisciplinary care teams
• Conduct wound care, oversee medication administration, educate patients and caregivers, and monitor ongoing disease progress
• Communicate essential updates and any change in patient condition to clinical partners
