We are seeking a Patient Care Manager responsible for overseeing patient care quality and ensuring compliance within a healthcare setting.
Key Duties Include- Providing clinical supervision to ensure care aligns with standards, policies, and regulations
- Coordinating and monitoring patient care for continuity and quality
- Supervising clinical staff to ensure consistent service delivery
- Maintaining effective communication with patients, caregivers, referral sources, and staff
- Driving quality outcomes through departmental goals and quality improvement initiatives
- Modeling professionalism, service excellence, and organizational values
- Ensuring compliance by completing training, accurate reporting, and cooperating with audits
- Promoting a safe work environment by following safety protocols
- Supporting staff development via coaching, mentorship, and ongoing education
- Current registered nurse licensure in the practicing state
- Experience in home care with at least two years of management or supervisory roles preferred
- Knowledge of professional standards, Medicare Conditions of Participation, and regulatory requirements
