Registered Nurse – Home Health

Requirements:

• Current state license as a Registered Nurse
• Homecare Homebase Software Experience
• OASIS Experience
• 1-2 years experience as a Registered Nurse in a clinical care setting
• 2+ years Home health experience
• Proof of current CPR certification
• Current driver's license
• Must have reliable transportation and insurance

Roles & Responsibilities:

• Assess patients in home health and/or hospice settings to identify physical, psychosocial, and environmental needs.
• Develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
• Educate patients and families regarding care, medications, and home safety, using age-appropriate teaching methods.
• Supervise and provide clinical direction to home health aides and other support staff.
• Manage pain through assessment and utilization of effective techniques, educating patients about pain management.
• Facilitate communication among patients, families, and the interdisciplinary team to coordinate care effectively.

Compliance / Notes:

• Must meet mandatory continuing education requirements.
• Maintains confidentiality as required.
