Registered Nurse (RN)- Home Care Visits

Schedule:

Part Time approximately 20 hours/ week. Flexible scheduling Monday-Friday. Potential for more hours.

Location:

North and South Metro Pay rate: $45-$55/ hour depending on experience plus mileage reimbursement

• Perform assessments/follow up assessments and procedures, match employee skills with the clients' needs and develop working relationships with discharge planners and social service workers.
• Obtain physicians plans of treatment and initiate execution, and provide services ordered by the physician as indicated in the care plan.
• Counsel and educate the patient/family in meeting healthcare related needs, and effectively communicate with members of the interdisciplinary healthcare team.
• Complete timely and accurate documentation/clinical notes including addressing the patient's progress.
• Supervise paraprofessional and licensed professional team members (LPNs). Secure equipment and supplies and coordinate placement for use in patient's homes, and refer patients to other services as needed.
• Participate in Quality and Performance Improvement (QAPI) program, along with the Office's Emergency Preparedness program as requested.
• Follow accepted standards of nursing practice and use clinical decision making to efficiently and effectively manage an assigned patient population.
• Access personal health information (PHI) necessary to fulfill the above duties and responsibilities.
• All other duties as assigned.

• An active RN license in the states of MN.
• One year of experience in Home Health
• Current CPR/ BLS
• Must have reliable transportation for patient visits including insurance

• Health, Dental and Vision insurance for full time employees (32 hours/ week or more)
• Earned sick and safe time
• Free on line continuing education courses
