Physician Assistant ​/ Nurse Practitioner - Transplant Surgery (1.0 FTE

Hennepin Healthcare (HHS) has a position open for a Transplant Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to work with adult populations (no pediatrics). This is a full-time (1.0 FTE) position working Monday-Friday with no call/evening/weekend requirements. This position involves working with the HHS (MNHC) Transplant Program. Emphasis is on, but not limited to, the kidney transplant population. The Transplant APP evaluates and manages kidney transplant candidates, recipients and donors as part of the multi-disciplinary transplant team.

This includes treatment of acute and chronic conditions in the inpatient and ambulatory care setting. The Transplant APP will also assist in dialysis vascular access surgery clinic.

• Completing pre-operative history and physicals, consents, orders for living and deceased donor kidney transplant candidates/recipients and living donors
• Collaborating with multiple disciplines in management decisions and care of the patient in preparation for surgery and final approval for transplantation or donation
• Coordinating and scheduling surgeries as directed by Transplant Surgical Staff
• Managing hospital admissions and discharges of transplant patients
• Providing post-transplant inpatient consults
• Performing comprehensive and interval physical assessments and completing daily progress notes on all assigned inpatients
• Establishing medical diagnosis for and management of acute and chronic problems
• Performing and interpreting laboratory and radiological studies
• Working closely with surgical resident teams to provide consistent care
• Serving as an educational resource for surgical resident teams which includes delivering an annual presentation on transplantation and maintaining transplant educational binder in resident lounge
• Providing support and education to nursing staff, patients and care partners
• Functioning as a liaison between multiple team members and specialties
• Providing coverage in dialysis vascular access surgery clinic for patients under evaluation for or in follow-up of access placement with staff surgeon supervision
• Performing pre-operative history and physical on patients requiring urgent vascular access surgery
• Working with the Transplant Multidisciplinary Team on quality projects
• Participating in discussion and revision of transplant protocol and processes

• Master’s degree from an accredited Physician Assistant or Nurse Practitioner program
• BLS/ACLS
• Independent with ability to collaborate with multiple disciplines and team members
• Strong communication and coordination skills
• Desire to work in a resident-based teaching hospital
• Genuine interest in chronic kidney disease/kidney transplantation
• Commitment to providing exceptional care

• More than 4 years of experience as a kidney transplant APP
• Interest in general surgery and dialysis access
• Interest in kidney transplant research
• Interest in resident and nursing education

Nurse Practitioner or Physician Assistant pay is based on year of licensure and specialty area. The grid below represents current pay rates (rates adjusted annually in April) for this position:
