** Position Summary*
* We are seeking an experienced  
** Travel Registered Nurse (RN)
** to provide compassionate, patient-centered hospice care throughout the Greater Lake Charles area. The RN will case manage a group of hospice patients, coordinate care, and ensure patients and families receive high-quality support throughout their hospice journey.

** Key Responsibilities*
* + Case manage an assigned group of hospice patients.

+ Provide compassionate, patient-centered nursing care.

+ Assess patient needs and coordinate appropriate hospice services.

+ Communicate effectively with patients, families, physicians, and the interdisciplinary care team.

+ Monitor patient conditions and respond appropriately to changes in status.

+ Maintain accurate and timely clinical documentation.

+ Follow hospice policies, procedures, and applicable standards of care.

+ Travel between patient visits throughout the Greater Lake Charles area.

+ Mileage between patient visits is  
** reimbursed** .

** Required Qualifications*
* + Current  
** Registered Nurse (RN) license** .

+ Previous  
** hospice nursing experience required** .

+ Strong clinical assessment and case-management skills.

+ Ability to work independently while collaborating with an interdisciplinary team.

+ Patient-centered approach with excellent communication skills.

+ Positive, flexible, and willing attitude.

** Preferred Qualifications*

Experience with  
** Homecare Homebase (HCHB)** .

+ Previous travel or community-based nursing experience.

* ** Pay Details:**  $55.92 per hour
