Twin Cities Physicians is seeking an On-Site Triage LPN to support providers for 4,500+ seniors across Minnesota in a non-patient-facing, office-based role. You will triage via phone, manage orders, and coordinate care, enabling timely and compassionate service.
Ideal candidates hold an active Minnesota LPN license with prior primary care or geriatrics experience, strong communication skills, and proficiency with EMR systems such as PCC or Eldermark. Monday–Friday schedule, no nights or weekends.
