Your visits can help identify problems earlier, improve patient understanding, prevent complications, and keep someone safely at home instead of returning to the hospital.

A Territory Built Around the Western Metro

You'll provide skilled nursing visits throughout communities in the western Twin Cities and Lake Minnetonka area , including areas such as:

Minnetonka, Wayzata, Mound, Orono, Delano, Watertown, Victoria, Carver, and surrounding western communities.

Rather than working inside the same building every day, your work takes you directly into the communities where your patients need you.

What You'll Be Doing

  • Provide skilled home health nursing visits in patients' homes
  • Complete comprehensive nursing assessments and ongoing evaluations
  • Perform wound care, medication management, injections , and other skilled nursing services
  • Monitor acute and chronic medical conditions
  • Identify changes in condition and intervene appropriately
  • Educate patients and caregivers about medications, diagnoses, treatments, and disease management
  • Coordinate care with physicians and other healthcare providers
  • Collaborate with PT, OT, SLP, social work, and interdisciplinary team members
  • Help patients safely transition home following hospitalization or rehabilitation
  • Complete timely documentation using Homecare Homebase (HCHB)
  • Help reduce unnecessary emergency department visits and rehospitalizations

Who We're Looking For

  • Active Minnesota Registered Nurse (RN) license
  • Previous experience in home health, hospital, acute care, rehabilitation, skilled nursing, or another clinical environment
