Case Manager – Hennepin & Scott Counties
Job Type: Full‑time
Overview: Pinnacle Services invites friendly and self‑starting professionals to join our Case Management team serving clients in Hennepin and Scott counties. With a person‑centered approach and a collaborative work environment, you will work with consumers across a range of needs, including developmental disabilities, serious mental illness, chemical dependency, and aging.
Responsibilities- Assess the areas of the client’s life where assistance is needed.
- Offer and coordinate resources for the client.
- Monitor service delivery and collaborate with providers to ensure client needs are met.
- Develop service plans.
- Attend regular meetings with clients and their interdisciplinary teams.
- Case‑note client work to create billable units.
- Maintain a caseload split about two‑thirds in Hennepin County and one‑third in Scott County; travel between counties as required.
Non‑LSW: $49,000 per year (salaried, 40 hrs/week). LSW: $51,600 per year (salaried, 40 hrs/week).

- Bachelor’s degree in social work, psychology, sociology, or a closely related field (or any field with relevant experience).
- At least one year of full‑time experience as a social worker, case manager, or care coordinator in a public or private social service agency.
- One year of experience working with persons served with developmental disabilities or related conditions.
- Valid driver’s license, acceptable driving record, and a vehicle with current insurance.
- Successful completion of required background checks.
- Licensed Social Worker (LSW) or higher BOSW license.
- At least one college course focused on developmental disabilities, verified by a transcript.
